If you have lost or damaged your licence, then you will be required to request a Duplicate of your licence. Note, you will be required to lodge a report with the Guyana Police Force, or, alternatively, an Affidavit of Lost or Damaged Licence can be obtained from a Justice of Peace (JO) or Commissioner of Oaths, before starting the process of applying for a duplicate licence.

Requirements for applying for a duplicate licence:

  1. A written report from the Guyana Police Force indicating the Licence lost or damaged; or An Affidavit from a Justice of Peace or Commissioner of Oaths.
  2. Taxpayer Identification Number (TIN)
  3. Copy of valid form of identification (National ID; Passport)
  4. One passport-sized photograph

The cost of a Duplicate Licence is G$5,000.00
